Available form types and their descriptions

Form Type	Description
New Patient Registration	For new patients needing to register as a patient with the U-M School of Dentistry. Includes: Registration Form, ID and Dental Insurance card image uploads form, Health History form.
ID and Dental Insurance Uploads Form	For new or returning patients only needing to upload their ID and/or Dental Insurance card images. The patient must receive a link to this form or a registration ID from Patient Services to use this form.
Health History Form	For new or returning patients only needing to complete the Health History form. The patient must receive a link to this form or a registration ID from Patient Services to use this form.
